the date, and the total amount paid for said purchase.

• Enter the receipt number or invoice number found in the receipt.

• Receipt will be invalid if you duplicate multiple entry of the same receipt

• Receipt should not be tampered, or digitally altered in any way.

Any receipt found tampered with will be rendered void and

consumer will automatically be disqualified.

• Required purchase should be at least worth

Php149.00 (single-receipt only). Any receipt showing

amount below Php149 will not be honored.
